SENSOR REPLACEMENTS

Due to Manufacturer guidelines, if a sensor fails for any reason, including falling off early, damaged items, being removed by a medical professional for a procedure, or malfunction the replacement must come from the manufacturer themselves as this allows them investigate what caused the malfunction. 

FreeStyle Libre Sensor Replacements

In order to request a replacement sensor from Abbott, the manufacturer of the Freestyle Libre, please either call the customer service team at 855-632-8658 or you can fill out a form online at https://www.freestyle.abbott/us-en/support/sensorsupportrequest.html and they will be able to coordinate your replacement. 

Dexcom Sensor Replacements

If you have a Dexcom G7 and require a replacement, please go to https://dexcom.custhelp.com/app/webform where you will find links to the sensor replacement form. You can also go to https://www.dexcom.com/contact to find the replacement form link as well as the self-help product support and the customer service chat and call queue.
